Jerry was born in Boone, Missouri and at the age of 70 he passed away in Fullerton, California. Jerry is survived by his loving wife Pamela, children Chad Douglas and Stacey Kolzow, 11 grandchildren 4 great grandchildren, daughters in law Kristine & Courtney and he was preceded by his son Jeremy Douglas.
